Dear parents, 

We wanted to include the following information regarding medical forms due to the school for the upcoming academic year. 

• Form I, Form III, and Form V students, and all students new to Priory: Must submit a new physical, regardless if they sent one in last year or not.
• Form II, Form IV, and Form VI students: Must submit the MSHSAA Medical History form. 
• Form I students: Must submit their current vaccination forms. Form II and Form VI students are likely to have to submit updated vaccination forms, depending on the age of the student.

All necessary forms are available on MyPriory. Those high school students participating in fall athletics must have a valid physical submitted to Mr. Brotherton or Mr. Neuner by his first practice on Monday, Aug. 8. The physical must be signed by a medical doctor, physician assistant, or nurse practitioner, and can be obtained at an urgent care, if needed.

All prescription medications must go to Mrs. Lane in its original bottle, with the student’s name, date of birth, and prescription dosage information listed, and a note from the doctor allowing Priory to give your student his medicine. All rescue medications should be accompanied by a signed action plan. Students are allowed to keep an inhaler in their locker or on their person.
